In order to better serve you, please acquaint yourself with the following description of our office financial and insurance policies.

Insurance
With the increased number of dental insurance, prepayment, and reimbursement programs, it is difficult to have a complete and accurate knowledge of all available programs.

For example, some companies provide dental benefits, but not orthodontic coverage, while others have age limits or different rates of coverage for orthodontic treatment. Some companies have stated TMJ coverage while others explicitly deny any TMJ treatment. Others have deductibles, and may cover only a percentage of the fee up to a maximum amount. We have also encountered plans that have yearly maximums that encourage patients to extend or delay treatment to obtain certain benefits. This last example may not only be unfair to the patient but can also affect treatment results. As you can see, it is impossible for us to know exactly what your particular insurance plan will or will not cover. For this reason, we recommend you get to know your insurance plan.

Know Your Coverage
The first step is to contact your personnel manager or benefits manager where you are currently employed. This person will be able to assist you in acquiring the appropriate information concerning your coverage, as well as to provide you with medical and/or dental forms. Many patients do not realize the effort we would have to put forth to interpret their plans in order to gain all of the benefits possible for them. This is especially true with all of the changing benefits from year to year. We would never want a patient to become angry with us for committing an "error" when their reimbursements did not meet their expectations. For this reason, we recommend that each patient thoroughly research, understand and maintain their benefit plans.

How Most Insurance Companies Work
Your insurance company will reimburse you in their customary way. Every insurance company is different in how and what percentage they pay, depending on the particular plan. For example, most insurance companies will cover 50% of the fee up to a lifetime maximum. The most common lifetime maximum is $1000. Even if your account is paid in full at the beginning of treatment, many insurance companies will only reimburse you on their set schedule. If your insurance status changes (i.e. job change) your dental/orthodontic benefits may stop at that point, regardless of what you have already paid. Orthodontic benefits are among the lowest (percentage) reimbursement in all of healthcare since orthodontics is deemed "elective." In fact, many families that have "dental insurance" do not have orthodontic benefits. The last thing we want is an unhappy parent when they find out that their reimbursements don't match their expectations after treatment has begun.

Medical insurance
Craniofacial pain, whatever the source (malocclusion, TMJ disorder, poor posture, etc.), is a medical condition. As such, your medical insurance will likely cover at least a portion of the cost of therapy in our office. Please keep in mind that there are many factors to consider when dealing with medical insurance companies, such as: in/out of network coverage, type of policy (HMO, PPO, etc.), deductible, TMJ coverage, etc. We will be happy to assist you in maximizing your medical insurance benefits, although ultimately, reimbursement is your responsibility. Your insurance is a contract between you and the insurance company.

Service Philosophy
Since service is part of our philosophy, we will be glad to assist you in gaining your benefits by submitting an original claim form with all appropriate information requested by the insurance company. If your company requires additional records be sent to them, we will also comply and directly bill your insurance company for records duplication costs.

Payments to Our Office
Except under unusual circumstances, our office does not accept payments from an insurance company. This is in your best interest since you already know all of the information about your appropriate coverage. You can take immediate action should any reimbursement be incorrect. All responsible parties, whether they have insurance or not, will be required to sign a financial arrangement form if making monthly payments. Our financial agreement is with you and not the insurance company and therefore you have the ultimate responsibility for payment of services rendered.

Financing
In order to make paying for your dental services as convenient as possible, The Dental Health Center has a number of payment options, including Visa, MasterCard, American Express, checks, debit cards and low interest financing through third party companies, Care Credit and Capital One. Our office administrator will be happy to assist you with any of your financial concerns.

Appointments, Cancellations and Emergency Care
We see all patients on an appointment basis. However, if an emergency situation arises, call us immediately and we will see you as quickly as possible. If you are unable to keep your appointment, 24 hours notice allows us to give your reserved time to another patient. In the event that you miss an appoinment without 24 hours notice, or if you are more than 15 minutes late for a scheduled appointment, you will be assessed a $25 charge.
